Common mistakes to avoid
- Stopping at expected sedation. A learner may accept drowsiness after diazepam without checking the breathing risk. Opioid use, sleep apnea, difficult arousal, or abnormal respiratory effort shifts the priority to respiratory assessment and prompt escalation.
- Missing dehydration as a lithium danger cue. Fluid loss can make possible lithium toxicity more urgent. New gastrointestinal or neurologic changes call for immediate assessment and prescriber notification, not reassurance or a routine follow-up plan.
- Expecting an immediate SSRI response. Lack of early improvement does not prove that the medication is ineffective or that the patient is nonadherent. The corrective approach is to assess the prescribed use, tolerability, worsening mood, and safety concerns without promising a fixed onset.
- Turning orthostatic symptoms into a medication-change order. Dizziness after standing is first a safety and assessment problem. Assist the patient, reduce fall risk, reassess as appropriate, and report findings rather than independently changing olanzapine.
- Waiting for severe infection signs with clozapine. Fever, sore throat, or mouth sores should trigger prompt reporting and verification of required blood monitoring. A prior normal result does not remove the need to respond to current symptoms.
- Using AIMS as a casual checklist. Tardive dyskinesia monitoring requires systematic observation and clear documentation of involuntary movements. Do not label every movement automatically, but do communicate new or worsening findings for evaluation.
