- Confusing synchronization with defibrillation. A high rate alone does not determine the shock. Confirm whether a pulse is present and whether the unstable rhythm is treated with synchronized cardioversion; a pulseless shockable rhythm calls for defibrillation and resuscitation actions.
- Over-reading an ECG finding. A prolonged PR interval or independent atrial and ventricular activity may be memorized as a label without asking what the nurse should do. Link every finding to pulse, symptoms, perfusion, monitoring, and escalation.
- Reducing atrial fibrillation to palpitations. An irregular rhythm can be minimally symptomatic while still requiring evaluation of stroke risk. Assess neurologic status and reinforce the prescribed prevention and follow-up plan rather than changing therapy independently.
