Recognize: New neurologic deficits, severe or changing back pain, altered bladder or bowel control, acute dyspnea, facial or neck swelling, confusion, marked weakness, or reduced urine output can signal urgent complications.
Decide: Prioritize airway, breathing, circulation, and neurologic status; keep the patient safe; obtain focused assessment data; and notify the appropriate clinician or emergency pathway without delay.
Avoid: Do not wait for a complete history, routine rounds, or independent confirmation when a new high-risk pattern is present. - Respond safely to chemotherapy toxicity
Recognize: New respiratory symptoms during therapy, severe mucosal injury, infusion-site pain or swelling, and unexpected changes in condition require more than routine comfort care.
Decide: Assess the timing and symptoms, follow the medication or extravasation protocol, protect the patient from further exposure as directed, and escalate promptly.
Avoid: Do not force a flush, continue a concerning infusion, or label new symptoms as an unavoidable side effect before assessment. - Protect tissue during radiation therapy

Its difficulty profile is 13 easy, 12 medium, and 8 hard items. The Bloom distribution is 13 remembering, 1 understanding, 11 applying, 3 analyzing, and 5 evaluating. This means practice should include factual retrieval, but also action selection and comparison of competing nursing responses.

Study tips
- Start with diagnostic practice.
Complete a representative set from the 33 questions before rereading the topic. For every item, record the treatment or emergency pattern, your selected action, and your confidence so guesses are reviewed as carefully as incorrect answers. - Use focused retrieval by decision pattern.
Study in short blocks for surgery, chemotherapy, radiation, immunotherapy, neutropenia, and oncologic emergencies. Make a five-column comparison table: treatment or emergency, key cue, immediate risk, first nursing action, and escalation or teaching point. Fill it from memory before checking notes. - Review rationales as error analysis.
For each missed or guessed item, write one sentence identifying the decisive cue and one sentence explaining why the safer action comes first. Separate recognition errors, prioritization errors, and knowledge gaps. - Retry with spacing.
Return to the same missed concepts after a gap, then answer a parallel item without looking at the rationale. Rebuild the comparison table from memory and revise only the entries that remain unclear. - Finish with mixed timed practice.
Mix treatment adverse effects, postoperative findings, port safety, neutropenia, and emergency cues in one timed block. Afterward, review whether you recognized urgency, protected the patient, and selected the next nursing action rather than merely recalling a term.
Common mistakes to avoid
- Waiting for a dramatic fever before escalating neutropenia concerns. A patient with reduced immune defenses may show subtle or evolving infection cues. The safety principle is prompt assessment and escalation for new concerning findings, using the ordered protocol and current clinical context.
- Calling every post-treatment symptom expected. New neurologic change, dyspnea, facial or neck swelling, confusion, severe weakness, or reduced urine output can represent an oncologic emergency. The correcting cue is a sudden, progressive, or function-threatening change that takes priority over routine teaching.
- Using one care plan for every treatment. Radiation care emphasizes protection of the treated field, chemotherapy requires attention to systemic toxicity and marrow suppression, and immunotherapy requires careful review of new inflammatory symptoms. Match the action to the treatment mechanism and the patient’s finding.
- Relying on memorized numerical cutoffs without context. Fixed numbers cannot replace assessment, trends, orders, and facility protocol. When an item gives a concerning symptom pattern, prioritize the safety action and escalation pathway supported by the stem.
- Restricting all movement after cancer surgery or ignoring new limb swelling. Prescribed gradual mobility can support function, while new swelling, tightness, pain, or neurovascular changes need assessment. Follow the surgical plan and report changes instead of choosing either complete inactivity or unrestricted exercise.
- Treating an implanted port as automatically safe. The device does not remove the need for aseptic technique or site assessment. The correcting principle is to protect sterile connections, avoid force, and respond promptly to redness, drainage, pain, leakage, resistance, or systemic infection cues.
