This live inventory contains 90 original PNLE-style practice questions for Health Assessment, and it was last updated August 12, 2026. These are practice questions, not actual, recalled, or leaked board questions.

Health Assessment covers health histories, physical examinations, anthropometry, vital signs, and clinical data collection. You practice recognizing whether information is reported or observed, collecting it in a reliable sequence, choosing safe measurement conditions, identifying an unexpected finding, and deciding when data need validation or prompt communication.

Health Assessment is a Tangerine pedagogical lens in NP1, Fundamentals. It supports relevant competencies across the official five-subject PNLE TOS; the 2025 Enhanced TOS does not assign a guaranteed microtopic count to this lens. Diagnosing and planning care belong in Nursing Process, while disease management is outside Fundamentals, so these items emphasize accurate assessment and safe next steps. Your goal is to produce trustworthy data before another nursing decision is made.

  • Separate reported data from observed data
    Recognize: Subjective data come from the patient or caregiver, such as a reported symptom or concern. Objective data are observed, measured, or obtained through examination.
    Decide: Identify the source of each finding and document both the patient’s report and your measurable observations when relevant.
    Avoid: Treating a patient’s description as a measured finding or dismissing it because it cannot be directly observed.
  • Build a purposeful health history
    Recognize: The reason for assessment, the patient’s main concern, timing, associated experiences, relevant history, and current treatments shape the questions you ask.
    Decide: Begin with open questions, then clarify details using neutral, focused questions and language the patient understands.
    Avoid: Leading the patient toward an answer, assuming missing information, or recording an interpretation as though it were the patient’s own statement.
  • Match examination sequence to the body system
    Recognize: Inspection, palpation, percussion, and auscultation are organized according to the system being assessed. Abdominal assessment uses a different sequence from many other examinations.
    Decide: For the abdomen, auscultate before palpation and percussion so the examination technique does not alter the finding you are trying to hear.
    Avoid: Applying one routine sequence to every body system or beginning with a maneuver that can change later assessment data.
  • Standardize vital-sign collection
    Recognize: Position, activity, emotional state, thermoregulation, equipment, measurement site, and patient cooperation can affect a reading.
    Decide: Prepare the patient, use appropriate equipment and technique, select a safe site, and record the conditions that affect interpretation. Recheck a questionable result when safe.
    Avoid: Comparing readings collected under different conditions as though they were equivalent or accepting a result obtained with clearly poor technique.
  • Use anthropometry as contextual data
    Recognize: Height, weight, and related measurements require consistent equipment, positioning, privacy, and attention to mobility or assistive devices.
    Decide: Obtain and document the measurement method and relevant conditions, then interpret the result with the patient’s overall assessment data.
    Avoid: Using one measurement alone to assign a diagnosis or overlooking how clothing, equipment, or inability to stand affects accuracy.
  • Validate unexpected findings before labeling them
    Recognize: An unusual temperature, pulse, blood pressure, visual-acuity result, or position-change response may reflect technique, equipment, patient condition, or a true change.
    Decide: Assess the patient, check the method and equipment, compare with available baseline data, repeat when appropriate, and communicate urgent concerns through the proper channel.
    Avoid: Jumping directly from one abnormal value to a disease label or delaying attention to a patient who appears clinically unsafe.
  • Adapt special assessments to the patient
    Recognize: Orthostatic assessment, visual-acuity testing, and heart-rate collection in a restless young child depend on explanation, cooperation, positioning, and safety.
    Decide: Explain the procedure, stabilize the patient during position changes, use an age-appropriate approach, and document the conditions that may affect validity.
    Avoid: Calling an incomplete or poorly tolerated test normal, or allowing the pursuit of a measurement to create a fall, distress, or other safety risk.

What to expect on the PNLE

The live inventory supports single-best-answer questions that ask you to classify subjective and objective findings, choose the correct examination sequence, prepare for a reliable blood-pressure reading, select a measurement site, or identify the safest response to an unexpected assessment result. Other items require interpreting visual-acuity data, carrying out a position-change assessment, obtaining a heart rate from a restless young child, or recognizing how thermoregulation affects temperature collection.

Its difficulty profile is easy=49, medium=21, and hard=20. The Bloom distribution is remembering=38, understanding=8, applying=19, analyzing=11, and evaluating=14, so practice should include both direct retrieval and decisions based on technique, context, validity, and safety.

  • Applying: select the correct technique or preparation for a patient situation.
  • Analyzing and evaluating: compare findings with context, identify an unreliable result, and choose the safest next assessment or communication action.

Exact topic distribution varies by exam form. The official 2025 Enhanced TOS relates broad competencies across the five official PNLE subjects; it does not guarantee a specific Health Assessment microtopic count.

Study tips

  1. Start with diagnostic practice. Complete a mixed Health Assessment set without notes. For every uncertain answer, mark the task involved: history, examination sequence, vital signs, anthropometry, or clinical data validation. This separates content gaps from reading or prioritization errors.
  2. Use focused retrieval by decision category. On a blank page, write the assessment sequence, the conditions needed for reliable vital signs, and the questions that clarify a history. Then make this comparison grid:

  4. Retry with spacing. Re-answer missed and uncertain items after a gap, without copying the original rationale first. Explain aloud why the chosen sequence or measurement condition protects data reliability.
  5. Finish with mixed timed practice. Combine Health Assessment with adjacent Nursing Process and Patient Safety and Emergencies items. Afterward, classify each error as recall, application, analysis, or evaluation, then target the weakest decision type in the next focused session.

Common mistakes to avoid

  • Calling a patient report objective because it sounds specific.
    The correction is to ask who supplied the information and whether it was observed or measured. A precise symptom description remains subjective data unless an objective finding separately supports it.
  • Using the same examination order for every body system.
    The safety principle is to protect the validity of the finding. Abdominal assessment requires auscultation before palpation and percussion; applying a general sequence without considering the system can alter what you assess.
  • Choosing a blood-pressure arm without checking patient factors.
    The cue is the patient’s history, condition, devices, wounds, or other site limitations. Select a safe arm and use consistent technique rather than assuming either arm is always interchangeable.
  • Accepting an unexpected vital sign as final.
    First assess the patient and review positioning, activity, equipment, and technique. Repeat or confirm the measurement when appropriate, while promptly communicating findings that may signal immediate risk.
  • Turning anthropometry or visual acuity into a diagnosis.
    These results are assessment data, not a complete diagnostic conclusion. Interpret them with context, document the method and patient response, and keep disease labeling outside this Fundamentals assessment task.
  • Ignoring cooperation and safety during special measurements.
    Restlessness, inability to maintain position, or distress can make a result unreliable and can create injury risk. Explain the procedure, support the patient, adapt the approach, and document when the test could not be completed reliably.
